Privacy and Terms
Yours and my right to privacy will be respected at all times. The Privacy And Terms Agreement below has been designed to ensure the privacy and dignity of both the consultant's and the client's medical information. Upon ordering the One Month to Victory program, you will have the opportunity to agree to ALL TERMS WITHOUT ALTERATION. Without your consent to this agreement in it's entirety, I regret that I can not offer you my services.
 
Privacy And Terms Agreement
 

I understand that I am entering into a voluntary medical consultative agreement with Sunny Sedlock O'Malley. I agree that any/all information I receive will be received as such, and that I, and only I, am responsible for any medical choices/decisions/results that follow based on the information received during this process.

I agree that any/all information exchanged is strictly confidential, and will not be presented via any medium without express written consent from Sunny Sedlock O'Malley and the participating client. Failure of either party to uphold this clause may result in appropriate legal remedies being sought.


I understand that I am under no obligation to seek the medical advice of any referred physician(s) or suggested medical protocol(s). Further, I understand that I am under no obligation to extend this clause past the one month term of the contract.

I hereby agree to all the terms of this contract and agree to pay the consultative fee of $99.00. Any follow-up services will be charged as ordered and the terms of the initial contract will remain in full force and effect.
